HPU Form: Context, Category, and Card Options

Overview

The Household Profile Update (HPU) feature exists to enable parents to verify and update a specific set of family information stored in a Veracross database. Each HPU Form must be configured for a distinct context (from which it will be linked). These contexts include Online Re-Enrollment + Enrollment, Re-Enrollment Only, Enrollment Only, Parents, Admission Portal, Alumni, Faculty/Staff, and Public. In each context, common categories and cards may be added to collect information that is relevant to the schools and to that context in particular. This article outline those options.

Family Information

The Family Information category is available in all HPU contexts.

Category Card

Description

My Personal Contact Information Names, phones, & emails
My Family has Changed A textbox for Parents to describe complicated Household changes
My Children’s Contact Information Names, phones, & emails
Change Primary Address Add or change the primary address for the Household
Change Primary Home Phone Add or change the primary home phone number for the Household
Add/Change Secondary Address Add or change a secondary address for the Household (example: summer home)
Manage School Communication Opt-out of email or text message (SMS) notifications
Update Employers Add, change, or remove employers associated with the household
Higher Education Add, change, or remove higher education institutions associated with the household
Citizenships & Passports Add or remove passport and citizenship information
Visas Add or remove vias information (issue and expiry dates and visa numbers)
Review Vehicles Add information for vehicles including make, model, and primary driver (among other fields)
Household Primary Email Add or change the primary email address for the Household
Billing Information Add or change the primary email address for the Household

Household Contacts

The Household Contacts category is available in all HPU contexts.

Category Card

Description

Contacts for my Children Add, edit, and remove emergency contacts, relatives, & other contacts
Pickup Authorization Add, edit, or remove people with permission to pick-up children in the household
General Communication Recipients Add or remove household members who are flagged to receive general student communication
Report Card Recipients Add or remove household members who can view student report cards

Medical Information

The Medical information category cards are available in all HPU Contexts.

Description

Family Insurance Information Add, edit, and remove household insurance provider, medical ID, group and subscriber numbers, and expiry date
Review Medical Profile This links to the Medical Profile page
Review Household Doctors Add or remove physician name and office contact information
Review Household Dentist Add or remove dentist name and office contact information
Medical Notification Recipients Add or remove household members to be contacted during a medical emergency for a child in the household
